Dx/bendix Kickback Washers

Hi,
I'm fitting a NOS Bendix red band 2 speed kick back to my 48 DX.
The sealed bag which came with the Bendix contains the brake arm strap and two flat/smooth axle washers and two axle nuts.
Is there anything else I'm going to need when installing the wheel to the frame? Serrated washers, etc? What is the correct order of parts when fitting?

Nut, washer, drop out, hub, drop out, washer, nut left to right or right to left. You do not need serrated washers. Roger
