How would the Elgin front fender ornament be applied over what appears to be a fender that looks
like it requires a light fixture?

The bike has Skylark fenders and rack. You would have to modify all parts to make correct for BB which is not too difficult. V/r Shawn

Maybe I'm wrong but I seem to remember this picture and discussion from about that time picture was from Craigslist original seller maybe not yours I urge caution.

I remember similar pictures from Dave's Vintage
Bicycle web site.
The bike was in primer grey.
But I can't say for sure if this is the same one.
