Bearings keep loosening up

Hi all. Has anybody set the bearings snug so there is no play only to find they loosen up after around 10 miles of riding?

The head and front wheel bearings on the western flyer and the head and crank bearings on the Iver Johonson.

Maybe they are too snug to start with.

I've had the problem with fork bearings, usually from the fork head tube being bent.

Missing the keyed lock washers, nuts, bad/worn cups, ball bearings, races and or cones.
