I do believe that I have what you are looking for. There is a crimp in the top left fork tube presumably from when someone had to pull the fork off the axle. It could be massaged back with some heat I would guess or use it as is. It has holes to accept the axles, not slots. It is straight but it looks like the head tube has had the threads chased.
