My best guess was early tandem or MC or maybe even one of those crazy TOC two speeds when I picked this up but I haven't seen anything similar in searching.
Looks heavy duty enough be early Motorcycle?
I included the more familiar cog for scale.

my 92 Victor 91 Triumph and both my 92 columbias cog and chain ring hover around 3/8 -1/2 inch
