Restored phantom question

I thought this was interesting. I have a restored ‘55 Black phantom and saw this taped to the inside of the tank.

Did/does Jerry Peters from Chestnut Hollow restore bikes? If so, could this be one he restored for someone named Bruce?

Have you checked with his son, Jerry Jr. @ Chestnut Hollow?
He helped me locate the E.A. Horn. A very nice person to talk to.
