Can't help with an ID but I sure like that design! That's fricken cool.
 
It goes on a Westfield double-D crank, often for Wards, in that ovals pattern, circa 1930.

Luxlow a member customizes and restores bikes, and can mix parts; I have seen R.S. designs with drive pins.

Some older double-D cranks have odd coarse 20 threads per inch hardware, and there are 1990s reproduction cranks of similar design, but common threads.
