TOC Tandem Identification Assistance Please!

It has been a pretty busy TOC tandem week for me as this one has been acquired for the collection; a few days ago I made a deal for another special Massachusetts built tandem I regretfully sold last year.
Can anyone identify the manufacturer of this bicycle? The rear axle adjustors and the straight leg four-plate front fork are very distinctive. It looks like it has been badged differently twice. I suspect maybe a second badge could have been mounted when the 20th century remodel occurred. Thank you for any help!
