Town and Country wheel options

Has anyone changed up the wheels on a T & C tricycle? I am switching to 36 hole aluminum rims and have found sealed bearing front hubs but am looking for a coaster brake multi-speed rear hub that is 36 hole. I don't want anything too complicated as my wife will be riding this and she wants something easy but it would be nice to have different gears for easier pedaling. Ideas? Anyone? Thanks for the help gang!
