Here is a higher-resolution crop of the photo showing just that bike:
It looks like the cranks are a bit out of phase. That setup would put a huge cantilevered load on the crank arm to crank spindle interface.

So I guess gear shifts go like this:
- Relax the chain tension by moving the lever first to the right to disengage the ratchet teeth and then forward.
- Begin spinning the cranks backwards.
- Turn the tab at the end of the lever in one direction to shift up or the other to shift down until the derailleur paddles have moved the chain to the desired sprocket.
- Stop spinning the cranks backwards
- Move the lever backwards to re-tension the chain.
- Resume pedaling forward.
